Free peroneal autograft for arophic pseudoarthrosis after failed osteosynthesis of humeral shaft fracture

Introduction: Surgical treatment of osteoporotic atrophic pseudoarthrosis of the humeral shaft after failed surgical fixation is considered challenging.
Objective: To demonstrate the efficiency of the free peroneal autograft technique in the repair of atrophic humeral pseudoarthrosis.
Case report: We report the case of a 70-year-old obese man who suffered a domestic accident and had a distal periplate fracture of his left humeral shaft. He was operated on by open reduction and internal fixation with a narrow 4.5 mm dynamic compression plate and screws. He remained in pain and significant functional disability. After the diagnosis of atrophic pseudoarthrosis of the humeral shaft, he was treated with free peroneal autograft and bilateral iliac crest cancellous bone.
Conclusions: Treatment of atrophic humeral pseudoarthrosis with bilateral cancellous free peroneal autograft from the iliac crest is considered an efficient method, as it guarantees consolidation with good clinical and radiological results, minimal morbidity and low economic cost.
