Surgical treatment of adolescent idiopathic scoliosis with transpedicular systems

Abstract

Introduction: Surgical treatment with transpedicular screws is considered the gold standard for adolescent idiopathic scoliosis. At the Spinal Surgery Department of Frank País International Orthopedic Scientific Complex, the high incidence of this condition has increased the surgical backlog.

Objective: To evaluate the results of surgical treatment of adolescent idiopathic scoliosis with transpedicular screws.

Methods: A quantitative, descriptive, prospective, longitudinal panel-type study was conducted from March 2022 to February 2023. The sample consisted of seven patients. Data were collected from medical records, follow-up visits, and SRS-22 questionnaires. The information was analyzed using statistical calculations.

Results: The mean patient age was 15 years, with a female predominance. A history of primary idiopathic scoliosis was present in 42% of cases, with Lenke subtypes I-A-N and III-B-N being the most common. A 75% correction of the Cobb angle was achieved. Instrumentation spanned T4 to L3 in 50% of patients, while hybrid constructs with hooks were used in two cases. The primary complications included superficial surgical site infections and moderate intraoperative blood loss, none of which resulted in severe or long-term sequelae.

Conclusions: Six months after surgery, all patients had a quality of life rated as very good or excellent.
