Introduction: Weber B fractures are one of the most common ankle traumas, and therefore there have been numerous studies on their management and surgical techniques. The definitive approach is controversial in terms of its long-term effectiveness in the patient.
Objective: To evaluate the efficacy of internal fixation on the posterior and lateral aspects of Weber B fibular fractures in patients at Víctor Lazarte Echegaray Hospital from 2018 to 2023.
Methods: A retrospective, analytical, observational cohort study was conducted. Seventy two patients were evaluated: 24 from the exposed group (posterior aspect) and 48 from the unexposed group (lateral aspect). The efficacy of both techniques was analyzed by comparing functional and cosmetic outcomes, as well as disability duration and late morbidity.
Results: Osteosynthesis treatment on the posterior aspect, compared to the lateral aspect, showed better functional and cosmetic results, shorter disability duration, and lower late morbidity (p < 0.05).
Conclusions: Plate osteosynthesis was more effective on the posterior aspect than on the lateral aspect.
