Deep gluteal syndrome

Introduction: Buttock pain, identified in various locations where sciatic or pudendal nerve entrapment can occur, is known as deep gluteal syndrome. This syndrome is characterized by extrapelvic compression of the sciatic nerve within any structure of the gluteal space.
Objective: To review information regarding deep gluteal syndrome, including the boundaries of the gluteal space, symptoms, diagnostic maneuvers, and possible treatments.
Methods: Recent publications on deep gluteal syndrome were reviewed to update definitions and clarify the symptoms of this condition. Articles from journals indexed in Medline database and relevant books were prioritized.
Discussion: Sciatic nerve entrapment in the posterior hip area causes pain in that area and the buttocks. The definition of deep gluteal syndrome is based on the signs and symptoms caused by non-discogenic entrapment of the sciatic nerve in the deep gluteal space.
Conclusions: Conservative measures recommended include rest, activity modification, medication, various therapeutic modalities, and targeted education. Patients with severe symptoms require surgical intervention, which involves decompression of the sciatic or pudendal nerves.
