Double-plating for comminuted osteoporotic proximal humerus fracture

Abstract

Introduction: Some proximal humerus fractures in elderly patients with osteoporosis are accompanied by medial metaphyseal comminution. Lateral Philos plate fixation is the most commonly used treatment. However, it cannot prevent loss of medial reduction and varus deformity, especially in cases of an unstable medial column.

Objective: To demonstrate that double-plating for proximal humerus fractures increases fixation stability.

Case report: We report the case of three female patients with Neer type 3 comminuted osteoporotic proximal humerus fractures, severe varus displacement, and medial column involvement. They were treated with double plating, and satisfactory functional and radiological results were achieved. Conclusions: Medial buttress plate fixation, in conjunction with the lateral Philos plate, is a viable option for the treatment of proximal humerus fractures in patients with an unstable medial column, varus deformity, and osteoporosis.
